EE hooked up to pull up dock plate and since strap was on which was thicker hook slipped off. EE states [REDACTED] noticed arm was pulling and there was large contusion and inflammation on bottom of elbow and RT arm. EE reported and was sent to UC to get X-rays. EE states was told to ice compress the area and elevate as well as avoid moving it. EE placed on light duty and need to f u with surgeon and MD on [REDACTED].
