Before the incident

The employee and their supervisor were opening Cavern [REDACTED] manually because the motor had failed broken. This took them approximately 1 hour with both taking turns cranking on the valve.

What happened

MOV-01 motor had failed on [REDACTED]. The employee and their supervisor (shift supervisor) took turns hand cranking the valve open for about an hour for the valve to completely open. Note: The valve turned easily but took a very long time to open. The employee completed their shift and went home. The employee called their supervisor at [REDACTED] reporting that their shoulder hurt. The supervisor contacted the site 's Control room operator.
