Before the incident

EMPLOYEE WAS TEST RIDING THE BLACK HOLE. THROUGH VIDEO FOOTAGE I OBSERVED HIM PROPELLING HIMSELF DOWN THE SLIDE AND THEN THROWING HIS RIGHT ARM IN THE AIR WHILE ENTERING THE FLUME. THE LEFT ARM WAS NOT VISIBLE ON CAMERA FOOTAGE.

What happened

EMPLOYEE HIT HIS HEAD ON THE INSIDE OF THE FLUME FOR THE BLACK HOLE. HE THEN SAT INSIDE OF THE BOWL FOR APPROXIMATELY 7 MINUTES BEFORE SCOOTING HIMSELF OUT OF THE SLIDE.
