EE states she was driving her forktruck thru the maintenance corridor where she attempted to go thru the fire door. The top of the fork truck hit the top of the fire door with impact throwing her approximately 5ft off her truck onto the floor. EE is complaining of left ankle left knee and left shoulder pain.
