EE alleges she has a headache and neck soreness. EE has taken OTC medications and applied ice. EE agrees to and verbalizes understanding of Self-Care Instructions for this injury; will call back if new worsening s s occur. EE agrees to inform supervisor of Recommendation & Medcor Reference number.
