Before the incident

The employee was using a robot coupe to slice mushrooms for a number of minutes prior using the plunger to process the mushrooms proper technique. He spoke briefly with someone next to him and then proceeded to use his hand to push the next batch of mushrooms down the feed tube instead of using the plunger like he previously had been.

What happened

The employee was using a robot coupe to slice mushrooms for a number of minutes prior using the plunger to process the mushrooms proper technique. He spoke briefly with someone next to him and then proceeded to use his hand to push the next batch of mushrooms down the feed tube instead of using the plunger like he previously had been. In doing so he put his hand too far in and amputated the two tips of his fingers in the blade. He had been using proper technique with the plunger so I have no idea why he stopped.

Amputation of the tips of two fingers. Portions of the nail remains so maybe 1 4 inch or so.
