Replacing bolts on automatic fence

What happened

A Maintenance Technician was replacing bolts that had come loose in the automatic gate drive unit. He had the lever pulled to disengage the chain from the motor but the lever failed and when the gate started to close he got his fingers in between sprocket and chain causing puncture wounds on the right middle finger and removed skin on right pointer finger.
