The EE was inventorying pallets visually to verify the physical location and item to the data in the system. As the operator of the High reach elevated this EE while they were in a caged platform. to the 3rd and 4th level. The EE stepped off of the platform and out of the cage onto a rack to get a closer look at the pallet label. The EE was not following protocol and did tether to an anchor but was Wareing their fall protection harness.

The EE tried to return to the platform but slipped and fell 20 feet to the floor surface.

EE sustained a serious head should and rib cage injury that resulted in their death.
