What happened

Employee went into residents room to provide care. She introduced herself & informed patient that she was there to provide care. Once she laid the resident flat the resident became combative so the employee stepped into hallway & asked a co-worker to assist her. When they returned to the room & proceeded with care the resident became even more combative & started hitting grabbing & scratching. The resident grabbed the employees arm and dug her fingernails into her forearm breaking the skin & causing cuts.
