Before the incident

Operator was working to correct a leaking nitric hose at the ep line.

What happened

While doing so the operator forgot to don the appropriate chemical PPE. The operator was going to turn a valve and did not realize he was actually turning on the valve. This allowed nitric to flow out of the hose and contact the operator.
