Employee was helping set wall forms. He was at the base of the wall form while another employee helped lift the panel into place with a forklift.

The chains on the lifting device were set too short so in order to lower the wall panel all the way down the operator tipped the forks down. In doing this the lifting device slid off the forks and landed on the employee.

Laceration and swelling of the left side of the face especially the cheek.
