What happened

While performing his duties The safety seal under the cap was removed from a 1-gallon bleach container (Figure 2 [REDACTED] containing 6% Sodium Hypochlorite) when the container slipped from his grasp. A droplet dislodged and splashed into his unprotected right eye (he did not wear PPE safety glasses).
