Before the incident
On [REDACTED] at approximately [REDACTED] Safety & Security [REDACTED] of [REDACTED] was informed by [REDACTED] Engineer [REDACTED] of an associate injury that occurred on [REDACTED]. The associate who sustained an injury was identified as [REDACTED] from PRT Maintenance. [REDACTED] explained she was attempting to relocate the nightstand between both queen beds when she suddenly struck her left foot against the bed frame. [REDACTED] stated the pain went away shortly after so she did not inform management or security of the incident. When [REDACTED] finished her work shift and got home she began to feel pain and noticed inflammation from the injury. The following day on [REDACTED] [REDACTED]s pain was still present in her left foot so she decided visit her personal Doctor at Kaiser Permanente to be diagnosed. Dr. [REDACTED] ([REDACTED]) of Kaiser Permanente took x-rays of [REDACTED]s injured foot and deemed her unable to work from [REDACTED] [REDACTED] (Work Status Report). Ac
