On [REDACTED] at approximately [REDACTED] [REDACTED] reported having pain to AGM Dilma [REDACTED] in the housekeeping office. [REDACTED] then brought [REDACTED] to the Safety and Security office to document his injury. I Safety and Security Officer [REDACTED] assisted in taking the report for [REDACTED]. [REDACTED] was invited into the security office to document his incident. I asked [REDACTED] about his injury he stated that it was a specific injury but pain he felt in two places. Right leg near the hip and groin area. [REDACTED] stated that he felt pain from doing his houseman duties pushing carts pulling and lifting heavy linen. Depending on what he was doing he experienced different levels of pain. I asked when he started to feel the pain he stated that he was not sure that he 's been experiencing pain for several months and weeks.
