Employee was going to shut the infloor chain off

Employee was going to stop the in floor chain. When crossing the in floor chain he slipped and fell to the floor twisting his left knee. As the night went on his left knee and left foot started to swell up making it hard for him to walk. When I came he told me about this and we called [REDACTED]. He spoke to the nurse and they sent him to urgent care. During the investigation the employee was observed running over the in floor chain when he fell. he fell approximately 3 ft.
