Kneeling between the feeder beams to adjust the strapping to seal a barrel

What happened

Employee did not announce to co-workers he was going to be kneeling between the feeder beams and did not lock out the inverter that tells the feeder beam to move when a barrel has been inverted. The feeder beam activated and pushed his knee body towards the box sealer which led to a bolt puncturing his right knee
