What happened

At approximately [REDACTED] on [REDACTED] two team members ([REDACTED] and [REDACTED]) were working together in the slip load area near the [REDACTED] desk. [REDACTED] was operating the Material Handling Equipment (MHE) while [REDACTED] was positioned alongside the pallet to assist in ensuring that the cardboard slip sheet aligned properly with the MHE to prevent the loss of product. As [REDACTED] began to move the MHE he did not notice TM on area and pinched TM finger with MHE as he engaged MHE
