What happened

Employee was preparing the reel tail for threading the winder. During the process a fold was created in the paper. The employee pulled the excess paper out and began to cut the tail. During this process she tripped slipped on a large wrinkle in the paper and fell to the ground. She attempted to catch herself with her hands resulting in a fracture to the right wrist.
