What happened

Employee was unpacking consumed material from the pallet to rack. one band was left uncut and wrapped around the pallet. Employee picks up small box from pallet and when going to place box on racking employee takes a back-side step to the right and right foot shoe gets caught on band and causes employee to lose balance and fall backwards landing on their butt and hands.
