I began work [REDACTED] at [REDACTED] leaving work on [REDACTED] around [REDACTED]. I tripped on floor mat just outside the employee entrance and fell on concrete.

I scrapped both palms attempting to catch my fall. Hurt my right hip and strained my right upper arm and shoulder. Sore back after the fall as well.
