Section 5(a)(1) of the Occupational Safety and Health Act of 1970:
The employer did not furnish employment and a place of employment
which were free from recognized hazards that were causing or likely
to cause death or serious physical harm to employees in that
employees were exposed to:
(a) The hazard of amputation or crushing injury. On or
about 7/9/93 an employee was crushed while attempting to
flatten down the aluminum coil sheet in an attempt to
feed the shear mechanism. A device was not installed onto
the machine, to prevent employees from having to manually
place the aluminum into the shear while it is in
operation.
Among other methods, one feasible and acceptable
abatement method to correct this hazard is to provide an
automatic feed to the machine to prevent the necessity
of having to manually feed the machine, with the hands
or with tools. One type of feed mechanism could be an
enclosed box, to prevent the product from "curling"
before it reaches the shear area.
