Section 5(a)(1) of the Occupational Safety and Health Act of 1970: On or about 5/3/88, the employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falling into the hydraulic elevator pit which could cause serious injury or death. The hydraulic freight elevator mechanical lock and contact on the first floor had a piece of cardboard wedged into the mechanical lock/contact device which made the device inoperative and enabled the hydraulic elevator car to move with hoistway doors on the open position. A feasible and acceptable abatement method among others, is to correct the hazard as per American National Standard Institute, A17-1, 1981 Section 111.12, not to permit to install a device which would render hoistway mechanical locks and electric contact inoperative. a- Hydraulic freight elevator, main building, first floor - Mechanical lock and electric contact was made inoperative by the insertion of a piece of cardboard wedged into the mechanical lock/contact.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: On or about 5/3/88, the employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falling into elevator pit which could cause serious injury or death. The hydraulic elevator was not equipped with a device which would prevent the elevator from moving if the elevator was not at the landing. A feasible and acceptable abatement method, among others, to correct the hazard is to install mechanical locks and contacts which would prevent car movement if doors are in open position as per American National Standard Institute A17-1, 1981, Section 111.1(b)(1), which states "hoistway doors shall be equipped with hoistway-unit system, hoistway door interlocks." a- Freight elevator, main building, second floor - Hoistway doors were not equipped with mechanical locks and contacts which would prevent car movement if doors are kept in an open position.

There was no method of reaching someone trapped in the elevator if the elevator for whatever reason became stuck at any height of travel. A feasible and acceptable abatement method, among others, as per American National Standard Institute, A17-1981, Section 110.10(d), which states that elevators shall be provided with hoistway door unlocking devices. a- Hydraulic freight elevators (2), main building and adjoining - Elevators were not equipped with a device to open locked doors in the event of an emergency.

The sight glass on all elevator doors were missing, exposing an opening in the hoistway door causing a shearing hazard. A feasible and acceptable abatement method, among others, as per American National Standards Institute, A17-1981, Section 110.11, which states "manually operated hoistway doors of the vertical type shall be provided with a vision panel." a- Hydraulic freight elevator, main building, all floors - No vision panel (sight glass) was provided on hoistway doors.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: On or about 5/3/88, the employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being struck by heavy door resulting in serious injury. Vertical parting doors of a hydraulic freight elevator were not equipped with straps or other devices which would make opening and closing of doors safe. Presently, doors were closed by pulling the bottom section which is at floor level. The action simultaneously allows the upper half to come down. As the upper door comes down, a condition is created which exposes the employee to being struck by the door. A feasible and acceptable abatement method, among others, to correct the hazard is to install straps on each parting door as per American National Standard Institute, A17-1981, Section 110.1(f), which states "hoistway door of elevators which can be operated from the landings shall be provided with pull straps on the inside and outside of the door. a- Hydraulic freight elevator, main building, all floors - No straps were provided on vertical parting door for opening and closing purposes.

Signs were not posted in the freight elevator indicating no passengers. A feasible and acceptable abatement method, among others, to correct the hazard as per American National Standard Institute, A17-1981, Section 207.5(a)(2), which states "a sign shall be provided inside the car and shall be located in a conspicuous position and permanently and securely fastened to the car enclosure." The sign shall read, "This is not a passenger elevator, no persons other than the operator and freight handlers are permitted to ride on the elevator." a- Hydraulic freight elevator (2) - No sign posed in the elevator stating no passengers.

A feasible and acceptable abatement method, among others, to correct the hazard as per American National Standards Institute, A17-1981, Section 207.3(a), is to provide a capacity plate and data plate permanently and securely fastened in place. a- Hydraulic freight elevators - all - Capacity and data plates were not provided in a conspicuous place inside the car.
