Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) On or about 01/28/03, and at times prior to that date, the chain driven gate on the heli-pad malfunctioned exposing employees to the potential hazard of being struck when passing through the gate from the heli-pad to the hospital. Among other methods, one feasible and acceptable abatement method to correct this hazard is to schedule a routine maintenance and inspection procedure to check the alignment of the chain drive mechanism and alignment of the electronic beam and reflecting plate.ting
