Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
On and prior to 5/30/97, employees were exposed to serious injuries during
the operation of the
15 ton overhead crane in the miscellaneous repair area of the Central
Maintenance and Planning
Department due to inadequate training of radio controlled crane operators.
Among other methods, the following are feasible and acceptable methods to
minimize the
probability of injury during crane operations:
(1) Provide training for all crane operators which is consistent with
Works Safety Order #3.
Specifically, the crane operators shall be trained in the correct
procedures for; a) controlling a
swinging load, b) safely moving a load, c) standard hand signals, and d)
daily
limit switch
check.
(2) Ensure that only trained employees operate the radio controlled crane.
(3) Enforce all the rules and regulations set forth in OSHA 29 CFR
1910.179 and Works Safety
Order #3.
Refer to ANSI B30.2.0 - 1967 for recommendations related to remote
operated cranes,
specifically, Chapter 2-3.
