General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by falling objects. Southeast Elevator Shaft a) A bundle of steel flashing was lifted approximately 30 ft over employees working on the 16th floor, of the skeleton steel structure. The suspended flashings slipped from their bundle and the single sling (choker), causing each of the approx. 25 pieces (6"x8"x10') to fall from the 18th floor of the structure. The material fell down to a location on the 16th floor where employees were working, striking them and forcing an employee down an opening created by material falling to the 15th floor. Among other methods, one feasible and acceptable abatement method to correct this hazard is to avoid carrying load over employees, also for personnel to stand clear of the suspended load. The load shall be securely attached to the hook by means of slings or other approved devices, and that the load is well secured and properly balanced in the sling or lifting device before it is lifted more than a few inches.
