Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
a recognized hazard of electric shock, burns or electrocution.
a) M/V Woodbine - Employees in the "Ready Room" had occasion to use the
120 volt duplex outlet that had the polarity reversed. Use of
defective tool could expose the employees to the possible hazard of
electrical shock, burns, and/or electrocution.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to rewire this fixture to meet the NEC or IEEE
codes, and to test all other outlets for compliance.
b) M/V Woodbine - Employees in the breakroom had occasion to use the
120 volt duplex outlet that had the polarity revised. Use ofa
defective tool could expose the employees to the possible hazard of
electrical shock, burns, and/or electrocution.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to rewire this fixture to meet the NEC or IEEE
codes, and to test all other outlets for compliance.
