Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to amputation/crushing/struck by injuries as a result of
exposure to pinch
points/moving parts at the rear of the shear and not effectively being
trained in the safe methods
of the shearing operation.
(a)  Finishing Department, Shear #4 - Cincinnati 2512 Shear Capacity 3/8",

23907;  Employee involved in the shearing operation was not effectively
trained
as to the safe methods of work as specified in the manufacturers
operation, safety
and maintenance manual and the applicable ANSI standard B11.4.  In
addition,
the rear of the machine was not equipped with an awareness barrier.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is
institute/implement an effective training policy to include all the
requirements in the Cincinnati
Mechanical Shear Operation, Safety and Maintenance Manual and the
applicable ANSI standard
B11.4.  In addition, install an awareness barrier in rear of machine per
manufacturer's
specification.
