Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment
and a place of employment which were free from recognized hazards that
were causing or likely to cause
death or serious physical harm to employees in that employees were exposed
to:
a)Star Kist Dock, in front of Scale #13 and Freezer 1: Each employee
working on the Star Kist dock is
not protected from the falling and crushing hazards associated with the
improper lifting of personnel
with forklift equipment.  On December 15, 2005, a worker died when the
scow he was directed to
enter fell off of the elevated forks of a rotatable forklift and crushed
the employee.
ABATEMENT NOTE:
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO
CORRECT THIS HAZARD IS TO:
1.TRAIN EMPLOYEES ON THE REQUIREMENTS FOR ELEVATING PERSONNEL
CONTAINED IN ITSDF B56.1-2005, AND
2.ENSURE THAT ONLY APPROPRIATE WORK PLATFORMS AND EQUIPMENT
MEETING THE REQUIREMENTS OF ITSDF B56.1-2005 ARE USED TO ELEVATE
PERSONNEL.
