Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to a recognized hazard of electric shock or burns:
(a)M/V Woodbine:  The outlet powering the water pump in the compressor room
tested as ungrounded.  This condition exposed employees who had occasion to
touch the pump to the hazard of electric shock.
Among other methods, one feasible and acceptable method of abatement would
be to ground the outlet.
(b)M/V Woodbine:  The cable providing power to the metal outlet box at the
aft end
of the machine shop was not secured where it entered the box to protect the
conductors from abrasion.  Employees who had occasion to use the outlet
were
exposed to the hazard of electric shock should the insulation on the
conductors
be abraded and the conductor be exposed.
Among other methods, one feasible and acceptable method of abatement would
be to secure the cord where it enters the box to prevent abrasion of the
conductors.
(c)M/V Woodbine:  The cord on the portable light in the propulsion room
had been
spliced and wrapped with electrical tape.  Employees using the cord were
exposed
to electric shock because the electrical tape does not provide the same
insulation
and abrasion resistance as the original cord insulation.
Among other methods, one feasible and acceptable method of abatement would
be to replace the cord on the light.

were exposed to amputation or electrocution if equipment on which they are
working is
reenergized:

(a)M/V Woodbine:  There was no effective means of preventing
re-energization of
equipment during maintenance of the processing equipment aboard the M/V
Woodbine.
Among other methods, one feasible and acceptable method of abatement would
be development and use of a lockout/tagout program.
Woodbine Alaska Fish Company was previously cited for a violation of this
Occupational Safety and Health Standard or its equivalent standard which
was
contained in OSHA Inspection Number 107108227, Citation Number 1, Item
Number 2, issued on 10/05/92.
