Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that:
On or before May 24, 1985, employees utilizing the South walkway,
Coke conveyor #4, were exposed to the hazard of falling from an
elevated level of approximately sixty (60) feet in height due to
inadequate securement of a walkway pad. The second walkway pad from
the top (west end) consisted of a reinforced concrete slab 2' 8-3/8
x 2'8 3/4" x 2"; the pad was situated atop support members (angle
iron/I Beams)  and located adjacent to an open area underneath the
conveyor.  No vertical member was in place between the pad and the
opening to prevent the pad from shifting northward due to vibrations,
etc., slipping off of the support members and falling to the ground.
Methods of abatement include but are not limited to:
1) Installing of a vertical member (clip) to prevent the likelihood
of the pad from moving northward.
2) Installation of a concrete pad, or alternate cover over the opening
under the conveyor designed so as to restrain movement of the walkway
pad northward.
