Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being injured by loads lifted with cranes and/or hoists. defects: Plant: 1.Shop made spreader bars with hooks were being used on the cranes to lift bundles of bar stock. They were not rated for the loads being lifted. 2.There were no recent crane inspections for cranes/hoists in the shop 3.Crane 8 (Production Equipment-Robins and Myers hoist) had the following defects: a.The lower load block on the right hoist did not have the capacity marked on it. b.There were defective wire rope cables on both hoists on the crane. c.The Pendant controls for the left hoist were not clearly labeled as to the function of each operating button. 4.Crane 7 (Robins and Myers) had a defective wire rope cable. 5.Crane 6 (P & H) had pendant controls that were not clearly marked. 6.Crane 3 (Production Equipment) had a defective wire rope cable. Among other methods the following are feasible and useful ways to correct the hazards: 1.Have a qualified professional engineer do a stress analysis of the spreader bars and their components (hooks and connections) to determine what the maximum safe lifting capacities are (with an appropriate safety factor). Rate the spreader bars based on the engineer's evaluation. 2.Develop a formal preventive maintenance program for cranes and hoists based on manufacturers' recommendations. This should include frequent (daily) inspections as well as periodic complete inspections. Maintain inspection records. 3.Ensure that all crane pendant controls are clearly marked. 4.Promptly replace defective wire rope cables when found on cranes. 5.Clearly mark load capacities on all crane and hoist load blocks.
