Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to:
a) being struck by 20" diameter hatch violently blown from its mounting by
air
pressure.
On Sept. 26, 1998, an employee was working near a 20" diameter hatch cover
while
an air pressure test was being performed in the rudder room below the main
deck of
the boat where the employee was working. The cover suddenly and forcefully
blew
off striking the employee in the face and chest inflicting fatal injuries.
Overpressurization contributed to this accident.
Among other methods, one feasible and acceptable abatement method to
correct
this
hazard is to formulate standard operating procedures to govern and control
the air
pressure testing work.
