North Carolina General Statute 95-129(1) of the Occupational Safety and
Health Act of North
Carolina:  The employer did not furnish to each of his employees
conditions of employment and
a place of employment which were free from recognized hazards that were
causing or likely to
cause death or serious physical harm to employees:
a)facility--a monorail hoisting system and supporting structure was not
provided
with an initial load  prior to being placed into operation.
REFERENCE:ANSI B30.16-1980; section 11-2.2 Testing, paragraph 11-
2.2.2 Rated Load Test.
NOTE:Among other methods, one feasible and acceptable method to
correct this hazard is to have the hoisting system and supporting
structure load tested.re
