Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish
a place of employment which was free from recognized hazards that were
likely to cause death
or serious physical harm to employees.  Employees were exposed to fall
hazards while working
from self propelled boom aerial lifts without personal fall protection.
a) 3201 Ashland, Chicago, Illinois - Employees were not provided with
personal fall protection
equipment for use while working from an Upright SP 37 aerial lift at an
elevation of
approximately 17 feet.
Among other methods, one feasible and acceptable method to correct this
hazard is to comply
with American National Standards Institute (ANSI), A92.5-1992 Standard for
Boom-Supported
Elevating Work Platforms and require employees to wear/use personal fall
protection anytime
they are in/work from an aerial lift.
Abatement documentation is required for this item in accordance with 29
CFR
1903.19 (d).
