Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of working under suspended loads: (A)Shingle Prep: On or about 12 December 2000, and at times prior thereto, two employees were working under suspended loads of roofing paper. The operation of the overhead hoist was not in accordance with the American National Standards Institute (ANSI) B30.16-1987, Safety Code for Overhead Hoists, section 16-2.3.2.3.f. Note: Abatement certification required for this item. ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD, AMONG OTHERS, TO CORRECT THIS HAZARD IS TO: Only hoist the load when needed, cordon off the area, and prohibit employees from entering the area below the load.

Section 5 (a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were standing and working from stepladders in an unsafe manner: (A)Shell Area: On or about 11 December 2000, and at times prior thereto, an employee was observed working from the upper most rung of a 4 foot stepladder. (B)Tape and Texture: On or about 12 December 2000, and at times prior thereto, an employee was observed working from the upper most rung of a 4 foot stepladder. (C)Exterior Siding: On or about 12 December 2000, and at times prior thereto, an employee was observed working from the upper most rung of a 4 foot stepladder. (D)Shingle Area: On or about 12 December 2000, and at times prior thereto, an employee was observed working from the upper most rung of a 4 foot stepladder. (E)Shingle Area: On or about 12 December 2000, and at times prior thereto, an employee was observed working from a 4 foot stepladder erected on a mobile scaffold. Note: Abatement certification and documentation are required for this item. ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD, AMONG OTHERS, TO CORRECT THIS HAZARD IS TO: Train employees on the proper use of stepladders and have supervisors check to ensure they are adhering to the training.

(B)Tape and Texture: On or about 12 December 2000, and at times prior thereto, the operator of the Toyota Powered Industrial Truck was not wearing the seat belt which was provided. Note: Abatement certification and documentation are required for this item. ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD, AMONG OTHERS, TO CORRECT THIS HAZARD IS TO: Train employees to always wear their seat belts and have supervisors check to ensure they are adhering to the training.
