Section 5(a)001 of the Occupational Safety and Health Act of 1970: The
employer did not
furnish to each of his employees employment and a place of employment
which were free from
recognized hazards that were causing or likely to cause death or serious
physical harm to
employees in that employees were exposed to a crane collapse:
On or about 08/21/03, employees were exposed to the hazard associated with
the collapse of the
crane tower and jib during the removal of tunnel forms.
Among other methods, one feasible and acceptable method to correct this
hazard is to insure that
the boom is not side loaded as per ANSI B30.5-1989/ASME B30.5-1994 Sec
5-3.2.1.4
