Section 5(a)1 of the Occupational Safety and Health Act of 1970:
The employer did not furnish to each of his employees employment and a
place of employment
which were free from recognized hazards that were causing or likely to
cause death or serious
physical harm to employees in that employees were exposed to a drowning
hazard when working
in a storm sewer:
On or about 9/16/05, it was determined that employees entered 10 foot deep
storm drains which
had approximately 8 feet of water in them, without personal protective
equipment, rescue
devices, or adequate training.
Among other feasible methods, one acceptable method of abatement was to
follow the American
National Standards Institute (ANSI) Z117.1-1995, Safety Requirements for
Confined Spaces.

REQUIRED FOR THIS VIOLATION.
