were causing or likely to cause death or serious physical harm to

were exposed to the hazard of being struck by the load:
a)On or about January 13, 2005 and February 9, 2005: Neither frequent nor
periodic inspections had been conducted on the Jet underhung hoist as
required
by ANSI B30.16 - 1981.
b)On or about January 13, 2005: Jet underhung hoist was missing the safety
latch
on the lifting hook.
Among other methods, feasible and acceptable abatement methods to correct
these
hazards are to:
1.Develop and implement a periodic and frequent inspection program for the
underhung hoist(s) in accordance with ANSI B30.16-1993, section 2.1.
2.Repair or replace the damaged safety latch in accordance with ANSI
B30.16-1993 Section 16-1.2.9.
