Stat. 182.653 subd. 2: The employer did not furnish to each employee, conditions of employment and a place of employment free from recognized hazards which caused or were likely to cause death or serious injury to employees: An employee was exposed to fall hazards while working from an unguarded platform, elevated by a rough-terrain forklift, without the use of a personal fall arrest system. Additionally, the platform was not provided with guardrails and was not attached to the forks or mast.
