Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to potential inhalation hazards of ammonia refrigerant: On or about 1/12/06 at and around the Machinery Room, employees were exposed to potential inhalation hazards of ammonia refrigerant due to: (1) an alarm set at a value not greater than the OSHA PEL (50 ppm) and a detector for ammonia leaks were not provided in the Machinery Room, (2) wiring methods did not conform to Class 1, Division 2, of the National Electric Code when the employer provided neither continuously run mechanical ventilation systems that would actuate alarms upon failure of the ventilation system, nor detectors located in areas where ammonia refrigerant from leaks would concentrate to actuate alarms and mechanical ventilation systems at 1,000 ppm ammonia, (3) piping, valves, and switches controlling ammonia refrigerant flow were not labeled and identified, (4) signage with the legend "Machinery Room - Authorized Personnel Only" were not posted at entrances to the Machinery Room, (5) schematic drawings or panels giving directions for the operation of the ammonia refrigeration system were not provided, and (6) emergency shut down procedures were not displayed in the Machinery Room. Among other methods, one feasible and acceptable abatement method to correct the hazardous conditions would be adherence to the safe design, installation, and operation of ammonia refrigeration systems as prescribed by ANSI/ASHRAE 15 - 2004 Safety Standard for Refrigeration Systems. **Abatement documentation required for this item**
