Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a falling or struck by hazard: Date: on or about 10/04/2006 Location: Line 5 - siding Equipment: Employer manufactured mobile scaffold Condition: An employee was observed working on a mobile (bakers) scaffold installing siding when a co-worker moved the scaffold horizontally while the employee was still on the working platform, which was approximately five feet above the ground. (Reference ANSI A10.8-2001 Section 4.8 and 11.3.3) One feasible and acceptable method to correct this hazard is to ensure that the employee is not on the working platform when the scaffold is being moved. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a fall hazard: Date: on or about 10/04/2006 Location: Line 5 - siding Equipment: Employer manufactured mobile (bakers) scaffold Condition: An employee was installing siding on a home that was being manufactured while while using a piece of wood that was supported on a piece of metal to increase his working height. (ANSI A10.8-2001 Section 4.33) One feasible and acceptable means of abatement method to correct this hazard is to work from working platform with out the need to use other objects to elevate the workers height. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
