Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees in
that employees were
exposed to:
Being struck by falling drill pipe, due to the fact that a cathead on the
drilling rig
drawworks had not been resurfaced so as to present a smooth surface for
the turns of the
hoisting line and  was still in use for hoisting purposes without having
defects removed
through the resurfacing of the cathead drum.
An industry consensus standard API 54, Recommended Practices for
Occupational Safety
and Health for Oil and Gas Well Drilling and Servicing Operations
specifies in paragraph
9.5.3 that catheads shall present a smooth surface.
Among other means, one method of eliminating this hazard would be to have
such
defects repaired by sending the cahead in to a machine shop, where the
cathead surface
is first built up by welding and then resurfaced by turning it on a lathe
as stated in API
9.5.3 of API RP 54.
