Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to a fall hazard:
(a)  Employees operating a Halla HLF 25C Forklift were using a maintenance
platform
that was 48" X 33" and 42" high that was not properly secured to the
lifting carriage or
the forks of the forklift.  The platform fell from the forks, on or about
10/05/06.
FEASIBLE AND ACCEPTABLE METHODS OF ABATEMENT INCLUDE:
-Follow the guidelines set forth in ASME/ANSI B56.1 - 2005 Standard for
Powered and Non-powered Industrial Trucks which states:
Section 4.17.3(b) - The platform attachment means are applied and the
platform
is securely attached to the lifting carriage or forks.
-Follow the manufacturer's (Modern Equipment Company) requirement that the
platform be chained to the mast of the forklift.
-Follow Chip Supply's own requirement contained in their Appendix for
Verbal
Forklift Personnel Carriage Training, dated June 2004, which includes the
requirement to "secure safety chain to carriage or forks".
-Follow the CAUTIONS listed by the manufacturer on the side of the
maintenance
platform, which includes the following requirement:  "Do not use the
platform
unless safety chain is secured to forklift".t:
