Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to a fall hazard while standing on the raised tines of a
forklift truck.
a) Worksite, Warehouse, at B-51B3 location - An employee was lifted to a
height of
approximately 15 feet, while riding on the forks of a powered industrial
truck (Yale,
Model NR035, Serial B815N05100Y). There was no platform, guard rails or
personal
fall arrest system in use; on or about 03/20/08.
ABATEMENT NOTE:  Among other methods, feasible methods of abatement to
correct this
hazard include, but are not limited to:
1) Enforce the proper application and use of fall arrest systems.
2) Utilize and enforce only platforms designed for elevating personnel
with forklifts (The
platform must meet the requirements set forth in ANSI/ASME B56.1-1969).
3) Train exposed employees about the hazard of falls associated with
improper elevating
methods.
Note: In addition to abatement certification, the employer is required to
submit abatement
documentation for this item in accordance with 29 CFR 1903.19
