employer did not
furnish to each of his employees employment and a place of employment

recognized hazards that are causing or likely to cause death or serious
physical harm to his
employees in that employees were exposed to:
(a) Auto repair shop: On or about March 30, 2010 and at times prior
thereto, employees
were exposed to a struck by and crushing hazard while elevating vehicles
using a Globe lift
with a non-functioning lock on a lifting arm.
(b) Auto repair shop: On or about March 30, 2010 and at times prior
thereto, employees
were exposed to a struck by and crushing hazard while elevating vehicles
using Globe and
Gilbarco lifts which had missing or damaged pads on the adjustable arm
extensions.
(c) Auto repair shop: On or about March 30, 2010 and at times prior
thereto, employees
were exposed to a struck by and crushing hazard while elevating vehicles
using a Rotary 4
post auto lift with damaged sheave bearing and shafts.
(d) Auto repair shop: On or about March 30, 2010 and at times prior
thereto, employees
were exposed to a struck by and crushing hazard while operating a below
ground, hydraulic
Weaver lift with missing locks, a pivoting front post, and extensive
hydraulic leaks.
Abatement Note:
Among other methods, one feasible and acceptable method to correct the
hazards noted is to
follow the requirements in ANSI/ALI ALOIM 2008 which include, among other
requirements, the following:
1.    Institute a preventive maintenance program according to the
manufacturer's
specifications.
2.    Provide training to employees on proper use and care of the lifts.
3.    Schedule and complete all periodic and annual inspections.
Abatement Note:  Abatement certification and documentation is required for
this item.
