Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: On or about June 26, 2008, during pressurized pipe line testing activities, employees were exposed to explosive forces and hazards including, but not limited to, the hazard of extreme noise exposure, sand blasting effects and crushing hazards due to a 20 inch diameter pressurized pipe exploding apart at the slip joint and injuring employees working in the area around the pipe. AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THE HAZARD WOULD BE TO ANCHOR THE PIPES PRIOR TO TESTING.
