which were free from recognized hazards that were causing or likely to

physical harm to employees in that employees were exposed to the hazards
of being struck-by,
caught-by and or crushed-by while working from the ground near a moving

On or about 07/16/2008, one employee was injured after being caught and
dragged under a
51,320 lbs front-end-loader while working near the equipment.
Among other methods, one feasible and acceptable method to correct this
hazard from occurring
again is for the employer to implement strict procedures which would
restrict employees to be
working in the presence of operating heavy equipment, which would be
consistent with following
the manufacturer's recommendation as stated in the Operation & Maintenance
Manual which
states "Keep bystanders out of the work area".
