Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to broken bones and/or possible death from a serious fall hazard while being elevated on an unapproved work-platform. a) Employees engaged in maintenance work were using an unapproved work-platform on the forks of a FGC25 Mitsubishi lift-truck and a 7700 Caterpillar lift-truck. The work-platform was not connected or attached to the forklift nor were the employees provided with adequate fall protection, such as a full body harness and lanyard exposing the employees to a fall hazard of up to fifteen (15) feet onto a concrete floor. Among other methods, feasible and acceptable means to correct these hazards are to ensure that work-platforms used in conjunction with forklift trucks meet the requirements of the American National Standards/Industrial Truck Standards Development Foundation (ANSI/ITSDF) Safety Standard for Low Lift and High Lift Truck, ANSI/ITSDF B56.1- 2005: 1) Section 4.17.2(c): Whenever a truck is used to elevate personnel, the following precautions for the protection of the personnel shall be taken: Be certain that required restraining means such as railings, chains, cable, body belt(s) with lanyards, or deceleration devices, etc., are in place and properly used. 2) Section 4.17.3: Whenever a truck is equipped with a work platform, the following additional precautions shall be taken for the protection of personnel: (a) Provide a platform that complies with the requirements in paragraph 7.37.3., and; (b) The platform attachment means are applies and the platform is securely attached to the lifting carriage or forks. In addition, repairing and using the scissors lift already in the company's possession to perform the necessary maintenance work. Abatement Certification and Abatement Documentation required.tion
