Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of anhydrous ammonia releases that can cause
chemical burns to the
lungs:
a) On or about February 25, 2009 - Anhydrous Ammonia Compressor Room,
pipes and
controls were not identified.
b) On or about February 25, 2009 - Anhydrous Ammonia Compressor Room, no
alarm was
provided at the entrance.
c) On or about February 25, 2009 - Anhydrous Ammonia Compressor Room, a
sign
reading  "Authorized Personnel Only" was not posted on the door.
Among other methods, feasible and acceptable methods to correct this
hazard
is to implement
the standards established by the American Society of Heating,
Refrigerating and Air-
Conditioning Engineers (ASHRAE), Standard for Refrigeration Systems
15-2001:
a) Section 11.2.2 - valves or piping adjacent to valves shall be
identified. Systems containing
more than 110 lb of refrigerant shall be provided with durable signs.
b) Section 8.11.2.1 - the alarm shall annunciate visual and audible alarms
inside the refrigerating
machinery room and outside each entrance to the refrigerating room.
c) Section 11.2.4 - each entrance to a refrigerating machinery room shall
be provided with a
legible permanent sign reading "Machinery Room - Authorized Personnel
Only".
