employer did not furnish employment
and a place of employment which were free from recognized hazards that
were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
wood dust explosion, deflagration, or
other fire hazards:
a.On or about April 14, 2010, in the old moulder building, the dust
collected by the Rees C/CX
enclosureless dust collector was not removed at least daily when in use.
ABATEMENT NOTE:  Among other methods, one feasible and acceptable method
to correct these
hazards include following NFPA 664, Standard for the Prevention of Fires
and Explosions in Wood
Processing and Woodworking Facilities (2007 Edition), Section 8.2.2.5.1.4,
including, but not limited
to, removing the collected dust at least daily or more frequently if
necessary to ensure efficient
operation.
b.On or about April 14, 2010, in the old moulder building, the dust
collected
by the Coral Antipollution
System enclosureless dust collector was not removed at least daily when in
use.
ABATEMENT NOTE:  Among other methods, one feasible and acceptable method
to correct these
hazards include following NFPA 664, Standard for the Prevention of Fires
and Explosions in Wood
Processing and Woodworking Facilities (2007 Edition), Section 8.2.2.5.1.4,
including, but not limited
to, removing the collected dust at least daily or more frequently if
necessary to ensure efficient
operation.
c.On or about April 14, 2010, in the moulder building where exhaust air
was recycled, the Air and Noise
Products dust collection system was not equipped with listed spark
detection, and the exhaust air ducts
conveying the recycled air back into the building were not equipped with a
high-speed abort gate
activated by the spark detector, an explosion diverter, or equivalent.
ABATEMENT NOTE:  Among other methods, one feasible and acceptable method
to
correct these
hazards include following NFPA 664, Standard for the Prevention of Fires
and Explosions in Wood
Processing and Woodworking Facilities (2007 Edition), Section 8.2.2.6.2,
including, but not limited to,
installing a listed spark detection system located on the duct upstream
from the dust collector and
downstream from the last material entry point, or on the exhaust side of
the dust collector, to detect fire
entering or occurring within the dust collector, respectively, and
installing a high-speed abort gate with
a manual reset activated by the spark detector to intercept and divert any
burning material to atmosphere
before it can enter the plant.
d.On or about April 14, 2010, in the saw building where exhaust air was
recycled, the exhaust air duct
conveying the recycled air from the Farr dust collection system back into
the building was not equipped
with a high-speed abort gate activated by the spark detector, an explosion
diverter, or equivalent.
ABATEMENT NOTE:  Among other methods, one feasible and acceptable method
to
correct these
hazards include following NFPA 664, Standard for the Prevention of Fires
and Explosions in Wood
Processing and Woodworking Facilities (2007 Edition), Section 8.2.2.6.2,
including, but not limited to,
installing a high-speed abort gate with a manual reset activated by the
spark detector to intercept and
divert any burning material to atmosphere before it can enter the plant.
e.On or about April 14, 2010, near the wood grinding room, the Industrial
Air Technologies scrap silo
was not equipped with either deflagration relief venting or an explosion
suppression system.
ABATEMENT NOTE:  Among other methods, one feasible and acceptable method
to correct these
hazards include following NFPA 664, Standard for the Prevention of Fires
and Explosions in Wood
Processing and Woodworking Facilities (2007 Edition), Section 8.10.3.2,
including, but not limited to,
installing either deflagration relief venting designed to relieve the
deflagration to a safe area andmaintain the pressure below the yield
strength of the vessel or an explosion suppression system.sion
